Physical Therapy- Initial Evaluation

Service Description

A patient must first have an evaluation performed before they may book follow-up treatment appointments. This includes new patients or an existing patient who wishes to have a new injury/body part treated. Initial Evaluation will consist of (1) a thorough health and medical history (2) social and lifestyle history (3) a comprehensive physical therapy evaluation (4) education on anatomy, pathology, diagnosis and plan of care (5) treatment including prescription of a home exercise program.
